fill in the form

This has been my understanding:
1. fill in the form - UK
2. fill out the form - US
Upon searching the phrase at US sites,
I see a mountain of 'fill in the form' examples
as below:

This one has both of the phrases 'Please fill in the
application and submit....' and 'Please fill out...'
on the same page.
I wonder if 'fill in the form' is rather frequently
used on the Internet because we are requested to
fill in a fewer and smaller boxes than in the case
of paper questionnaire, etc.
