Fill in, fill out, fill up, filled out, filled up

I wonder what right term should I use when I say, "I discovered that some of these forms were filed with some entries not filled ___.

should i say filled out?

I guess 'filled in' would be best in this context. Normally you fill out a form (provide all the answers) but fill in an entry. But sometimes they're just used interchangebly.
