When a phone goes off-hook without use for a set period of time, the receiver will emit a sound called the "off-hook alarm" or "off-hook" tone, which is much different from a busy signal. In the UK and old US systems, its a loud, warbling sound much like an ambulance siren. In the US, it's an annoying staccato tone, on and off at intervals of a tenth of a second.
