He fell down the stairs and died on the spot.

He fell off the stairs and died on the spot.

He fell down the full length of the stairs and died on the spot.

This one is:

He fell down the stairs and died on the spot.

"fall down the stairs" is the normal phrasing. "fall down the full length of the stairs" is an acceptable elaboration I guess. "fall off the stairs" is not usual. To me it suggests falling off a drop to the side of the stairs, not falling down stairs in the "normal way".
