Extraordinary leave request
I am having very crucial need of EOL on personal grounds to be on leave for three months from 18/03/2019 as my husband underwent a surgery, and this surgery would require proper attention and care for couple of months and also I am not having enough leaves.
Along with this request, I am attaching doctor letter for your kind review and reference. Could you please consider and approve my request for 3 months leave. ( from 18/03/2019 to 17/06/2019)
