As an American, I would use "he got an excellent grade on the annual exam."

I think you'd more commonly say something like 'the year-end exam'.

I think "in the end of year exam" may be more common in BrE. Though who knows.

("Annual exam" does suggest an exam that recurs year after year, with exactly the same questions.)

The terms "Quarterly exam", "Half-yearly exam", and "Annual exam" are commonly used Indian education system, mostly in elementary,middle, high, and higher secondary schools.
