00A known complication of LASIK in myopic eyes is regression. In our series the mean of spherical equivalent in the two groups was not different one month after LASIK surgery. But eyes with previous retinal detachment surgery had higher myopia 3, 6, and 12 after LASIK operation. These eyes developed -1.75 diopter of myopic regression in comparison of -o.62 diopter in eyes without previous retinal detachment surgery. This finding can be explained in several ways first the difference between means of spherical equivalent in the two groups, second effect of buckle and third subtle form of corneal ectasia. We used fellow eyes as control group. Diferrence that exist between the two groups in the degree of myopia can be explained by the encircling buckle effect. Most eyes with encircling buckles have a shift toward myopia. Myopic shift of -2.75 has been reported with this procedure4. A forward shift of the cornea and increased central corneal thickness are two suggested mechanisms of myopic regression after LASIK surgery.17-18 Similar levels of increased central corneal thickness were observed in eyes with and without retinal detachment surgery in our patients. Higher amount of myopic regression in eyes with previous retinal detachment surgery may be attributed to more forward shift of the cornea in these eyes. We think that outward forces of the encircling scleral buckle on a thin cornea after LASIK surgery may be the cause of this phenomenon. Another explanation is that subtle corneal ecstatic changes occurred in the eyes with previous retinal detachment surgery and resulted in observed myopic shift. We await results of long term follow-up to confirm or rule out this suspicion. In other similar reported studies10-11-12-20 myopic regression has not been addressed. 0-
