Yes. Some learners prefer to hold back in this way, while others sail merrily into speaking without reservation.

Not true in general. More common in men than in women in my experience, though there are exceptions on both sides.

Personally, in approaching a conversation-in-a-foreign-language task, I rehearse silently the first sentence or two to get "warmed up".

Yes, I do the same.
