I'd say that he is a 'referring doctor' and the patient is a 'referral patient'. Maybe Sri has a better idea.

1. The doctor who refers patients to the super-specialist is called a "Referral doctor".
2. The doctor who referred a patient to the super-specialist is called a "Referred doctor".
3. The patient who is being referred by a doctor to the super-specialist is called a "referred
patient".
