English proficiency recommendation letter

Hello everyone,I'm new on this forum and I was wondering if someone here could help.I'd like to apply for a London school of Economics summer course and they require appliants to provide an English proficiency certificate (such as IELTS or TOEFL) or alternatively, a recommendation letter from a teacher. My teacher ask me to write the letter which he'll sign later but i have got no clue how to write it.. I tried to look something on the net but apparently there are only letters from people introducing themselves.. How could i write it personating my teacher?Any help would be really appreciated!Thank you in advance!

Hello Mister Micawber,

I am a nurse from Spain and i need a letter to certificate my english level from my Matron to send NMC, she is so busy so i need to do it for her.
I dont know how have to be this letter they just gave me this basic guide:

The letter must be on official letter headed paper
It must demonstrate that you can communicate and interact with patients effectively
The letter should demonstrate that you are capable of communicating in English written format and that you are able to read English at a sufficient level.

I would like to know if you could help me or give me any idea about this.
