English names and surnames

how to distinguish English names and surnames except consulting a dictionary? As a rule, an English name comes before a surname, sometimes a middle name is placed inbetween, but it seems that the rule does not apply in all circumstances, I'm sure of that, but cannot give an example. And often, in some biblipgraphy of a book, sometimes name comes first with a punctuation, and then surname, sometimes the conventional order is reversed. Is there a rule? For example, we an author is cited, the formulation is often like this: "Qurik says",but in bibliophy llist, the name could be placed after surname: Randolph Quirk ...(or Quirk, Randolph) or maybe sometimes without punctuation?
