In my experience, Yours faithfully is seldom used today. I never use it.

Sometimes you know the name of the person you are writing to, and sometimes you don't. If you don't, then begin with Dear Sir (or with Dear Madam) . Some people suggest that Dear Sir should always be followed by Yours faithful
