Enclose with commas?

Hi. Can you please look at these and tell me if the commas are correctly placed for the parts underlined? If no. 3 is wrongly punctuated (with commas), do you have a sentence for me (so I can learn) where a phrase like the underlined should be enclosed with commas?

1. He did as told and as usual, he went home happy.

2. He swam two rounds at the olympic-sized pool, and before he got to do another two rounds, he took a short rest.

3. He went to the test site to take a test, and, as unprepared as he was, he took the test.

4. Although he wasn't ready for the test, thinking he could get by with things he already knew, he sat calmly in his chair to focus his mind.
