Subject: Request for tourist visa
My Name is ..., holding passport No … issued at department Jaffa Tel Aviv , date of issue: … I am 23 years old and I wish to travel to Italy for period 13 nights start from 04/06-16/06 for Tourism.
I was born in Ukraine and living in Israel from ... I need visa for reason that I don't have israeli nationality and my Passport is a Travel Document (see photo attached). I traveling with my boyfriend ... that don't need a Visa to Italy.
I am working in ... in Tel Aviv for past 2 years in ... .
I take over liability regarding all trip financial expenditures for the trip to Italy during the period.
In past 2 years I traveled to Germany, Spain and Turkey.
And now i wish to visit the beautiful Italy.
