DEAR SIR/MADAM,



Failed To Attend Examination



With regards to the above matter, I am <<name>>, father of <<name>>, 4th year dental student would like to inform the head of departments stated above that my daughter was failed to attend her semester examinations held on 18th and 19th November due to illness.



2. The examinations that she failed to attend were conservative dentistry(DCED 4101), Children’s dentistry and Orthodontic(DKED4101) and Periodontology (DEED 4101).



3. Together with this letter I am enclosing a copy of the medical certificate stating a medical leave of two days from 18th to 19th November.
