You are quite new to the Forums, so you may realize that we do not write compositions, essays or letters for our students. This is largely because you will learn much more by making the first effort yourself. Then, we will be glad to help you with any problems you may have.

Dear Sir/Madam:
I would like to request permission....

He already gave me permission last week, but I want to thank him for his kindness.

Do you call your boss 'Sir'?

This is a very culture-dependent matter,

In Canada, we typically call our boss by his name,
eg Mr Smith
or even
eg Tom.
