I think (a) would be fine. But how about (b)?
(a) Don't make the same mistake again.
(b) Don't make the same mistake twice.

Broadly speaking, yes.

But you could say A to someone who has already made the same mistake 100 times.
B would seem odd. It is usually said after the first mistake,
