Does this look straight and not polite?

Hi can you please tell me if the following sentence is polite and respectable?

I am sending this to our SVP so that asking your help guys. I have already sent him a formal relieving letter but was going through some problems with HR dept, yesterday I talked with through phone and he said yes. So I need to send an email directly to him copying the HR dept.

My email contents goes as below...

"As per my call with you, can you please help me to get myself formally relieved from all my duties in ABC Inc by approving on my reliving through this email to the respective persons CC'ed in here?

Thank you so much!"
