Does a comma belong here?

Unfortunately, my last thread sunk to the bottom of the forum without being answered. This should be a relatively straight-forward answer. The sentence is "I am requesting a promt written response, signed and dated." Does the comma belong there? Should a comma be put before "written" and after "prompt"? Where do the commas belong? Yes, you need the comma after 'response'. I'm not sure exactly what to call the situation, but it is acting almost as the phrase 'which should be'. No other commas.
