My question is related with the use of do/does with a 'Who' question for example:

- Who does always wear a hat ?
- Who always wears a hat ?

- Who does Carol usually eat lunch with ?
- Who Carol usually eats lunch with ?

Which one is correct ? and how to know when to use do/does ?
