Hello, I have two questions about "do a test " and "make an examintaion/test/check/exam"

1. I would like to know whether the subject (He) means a docotor or a patient in these sentences.

(a)He does an eye test.

(b)He does an medical test.

2. When I'd like to say "A doctor conduct a medical examination", do you think these sentences are natural?

(a)A doctor make an medical examination.

(b)A doctor make an medical test.

(c)A doctor make an medical exam.

(d)A doctor make an medicak check.
