Do you mind...?

(1) "Do you mind opening the window?"

(2) "Do you mind me/my opening the window?"

(3) "Do you mind if I open the window?"

(4) "Would you mind if I opened the window?"

What are the differences between these four sentences?

For illustrative purposes, I'd be thankful for the paraphrases of each questions.
