As you know, it is a set phrase of greeting with no current meaning beyond that. Historically, I am sure that it has come through a variety of forms-- 'heighdy', 'how'd'ye do' and so forth-- which originally asked the listener for his current condition of health or activity: what we ask today in the form 'how are you doing?'
