SentenceA: I am self-motivated and responible.
SentenceB: I have the ability to work under pressure and work in a team.

SentenceC: I possess good planning and communication skills.

Linked sentence:

I am self-motivated and responible, have the ability to work unders pressure and work in a team, and possess good planning and communication skills.

You need the second 'am', because you have mentioned several other verbs in your sentence.

Personally, i don't like sentences like this that are basically just long lists.
