Does "disembodied voice" mean 'a voice without a living creature emitting it (a ghostly one)' or not necessarily?

It doesn't literally have to mean that. Often "disembodied voice" would just refer to the impression given (e.g. you could not see the source of the voice, or the voice did not seem to be emanating from an expected place).
