Direct Speech. Why is the first comma outside the quotation marks? Shouldn't it be inside?

Hi teachers,
I've founf this sentence in a book. Why is the first comma outside the quotation marks? Shouldn't it be inside?
“There’s someone at the door”, he said loudly, but the old lady did not understand.
