Direct questions to indirect questions

Hello,

In changing direct questions to indirect questions:

Q1: She asked him who had tekephoned?
Which one is correct?
A1-1: "Who did telephone?" she asked him.
A1-2: "Who telephoned? she asked him.

Q2: He asked her where she had worked before?
Which one is correct?
A2-1: Where did you work before?" he asked her.
A2-2: Where have you worked before? he asked her.

Q1: She asked him who had telephoned.

A1-2: "Who telephoned?" she asked him.

Q2: He asked her where she had worked before.

A2-1: "Where did you work before?" he asked her.
A2-2: "Where have you worked before?" he asked her-- This one is possible, but is not the expected.
