Consider 100 attacks.

often at night = 70 attacks at night, 30 attacks in the daytime.

always at night = 100 attacks at night, 0 attacks in the daytime.

were often / often were = the same meaning.

tired and hungry means both at the same time.

tired or hungry means 'either, but not both'.
