Difference between "without" and "except"???

Please look at the following question.

"No one else ___ the building janitor has access to Mr.McKenzie's office on the 10th floor"

A)without B) except

The correct answer is B)except. If there is no "else", it's said that I can put "without" as well.
But I couldn't perfectly understand what is the difference between "without" and "except(except for)".
Could someone explain to me the difference between them?

The workbook containing that question said if there was not "else", we would have used "without" as well.
Could you teach me more? I mean I want to know the reason why I can't use "without" there. Thank you for your additional teachings in advance.

I suppose it's just about possible to use 'without' meaning 'unless accompanied by' if we drop 'else'.
