Difference between followed by and proceeded to?

I have a serious doubt on the usage of these two words or phrase: followed by, followed to and proceeded to/by.

For eg when we say incident A followed by B, what doeas that mean A happened first or B??

A B C D E F G H

now if i have to say that D is before E and after C, what would i say in terms if followed and proceed???

Kindly help me guys.

I am pretty confused, this doubt may be look silly to all of you but for me its important

Looking forward for the reply.
