Did or was

Hi,

I'm confused in the following two sentences. I really don't which one of the following two is correct:

was he slapped?

Did he slap?

What you guys think? Which one is correct from your observation?

This is passive voice. The non-question form is 'He was slapped (by somebody)'.

Did he slap? This is active voice. The non-question form is 'He slapped (somebody)'.

What you guys think? Which one is correct from your observation?

Kids have been escaped. This makes no sense. You can't make the passive with an intransitive verb like this.

Consider a transitive verb like 'cook'. It makes sense to say 'Dinner has been cooked (by Mary).'
