Did or was

Hi,

I'm confused in the following two sentences. I really don't which one of the following two is correct:

was he slapped?

Did he slap?


What you guys think? Which one is correct from your observation?

This is passive voice. The non-question form is 'He was slapped (by somebody)'.

Did he slap? This is active voice. The non-question form is 'He slapped (somebody)'.

What you guys think? Which one is correct from your observation?
