hello..

if someone fell down..should i ask..did it hurt/ is it hurt?

They both work, but "Does it hurt" might be more appropriate. " Did it hurt when you fell? Does it hurt now?

They both work, but "Does it hurt" might be more appropriate.

"Is it hurt?" means "is it injured?"

it is "dummy it", the same as in "It's raining". There is no referent to any person or any body part.

It hurts when I walk. (Walking causes me pain.)

It hurts when I turn my head this way. (Turning my head like this causes me pain.)

I assume that these sentences refer to the physical injury or damage, like a cut, fracture, and bleeding.
