I don't consider them interchangeable at all.

"despise" means "hate very, very much".
"look down on" means "consider inferior" (said of another person).

Intelligent people may look down on uneducated people, but that doesn't mean that they despise them.
Rich people may look down on poor people, but that doesn't mean that they despise them.
