Deny- Simple Past

The sentence is- "Patient denied assistance with mobility"

It happened in past. So, the simple past sentence is intended here. Some people argue that it means someone has denied assistance to patient. Some others say that it would mean that the patient has denied assistance to mobility. I think the latter is right. What do you think?
