Declaration of faith and marital status?

Hi guys,

I have a quite unusual request this time. My aunt asked me to write a declaration of faith and marital status of her son who lives in Scotland and is going to get married. Have you ever had to write anything similar? Emotion: smile Please, help me with the word choice and structure, as I have never written a declaration of any kind in English and I don't know what the established expressions for this purpose are.

Although I did have a shot at it, I don't know if it's any good.

Declaration of Faith and Marital Status

I, ______________, (hereby) solemnly declare that, as of the date of this Declaration, my son, ______________, does not have a spouse and is of Catholic faith.

1) does not have a spouse or [remains/is] [single/unmarried] ?
2) hereby - in or out?
3) is of Catholic faith or is a Catholic

3) any other suggestions, please?

This is quite urgent so I'd appreciate a quick reply.

You can say, if applicable, ... is not now and has never been married

... is an active and practicing member of the Catholic church.

It's just turned out that the declaration must come from both parents so I'll have to rewrite it a little bit. Could you have another look at it, please? Which one is better? The one with two I's or with we?
