Decay or was decayed or is decayed?

I happen to find that I don't know how to use the word decay properly.


The dead body has decayed?

The dead body was decayed?

The dead body is decayed?

The dead body decayed?


I find it hard to determine if active or passive voice should be used. Is "decay" considered to be an active process done by itself even though we know that it is facilitated by other organisms or bacteria, or a passive process?


I also find it hard to determine if present, past, or perfect tense should be used and what will be their difference?
