Dear ...

When you write a business letter, can you start the letter with "Dear" like "Dear Smith"? When you don't know if the receiver of a letter is a man or woman, what do you write first?
Plus, in Japan, we use "san" after people's names. Some non Japanese people write to us with this. Is this Japanese word, "san", common for most English speakers? Do you understand if I write a letter to you, like, "Smith san"?
