day to day minute to minute

Is this natural? Does it mean that thing can change from one minute to the next?
How would you have said it?

"She is sick, this is as serious as it gets, we are minute-to-minute, she is fighting, we are working" .

"She is sick, this is as serious as it gets, we are day-to-day, she is fighting, we are working" .
