"Cry" word meaning

Hi. I have question about "cry" word, which has many meanings, but I'm curious especially two of them.
1. cry - to utter inarticulate sounds, especially of lamentation, grief, or suffering, usually with tears.
2. cry - to call loudly; shout; yell.

For example I have sentence:
"I cried with pain"
How the person whom I will say this, will know what I have in mind?
I have in mind first meaning, but the person may understood this as the second meaning. Am I right?

How to recognize, which meaning was used?

There's not really enough here to distinguish between the two meanings. Could be either one.

If you wish to make #2 clear, you can use "cry out."

He cried out for help.
He cried out in pain.

Otherwise, it could be just a whimpering.
