Does "a couple of" always mean 2, or maybe more?

E.G. The song is unresolved because it's straddling a couple of different keys without really committing to either.

Here, the 'either' clearly confirms 'two'.

On the other hand, consider what it means when a woman says 'I'll be ready in a couple of minutes'.
