It's always helpful if you give us context when you ask what things mean.

I assume that it means it was not the first time something ever happened, or the first one ever seen, made, discovered, etc., but the second.

You might use this if many, many of these things have happend/been seen/been made, etc. since the (very) first and second (ever) of ... w
