I agree with your first answer.

There is something wrong with the second sentence. The first phrase just does not fit at all.

A(As a result of) B(heavy) traffic, his flight C(left) by the time he D(got) to the airport.

As a result of heavy traffic, his flight had left by the time he got to airport.

(I think the above is what is wanted by the teacher.)

But the first phrase still is out of place.
I would write it this way.

Because of the heavy traffic, he was late. His flight had left before he arrived at the airport.
