Could you please help me and correct any mistakes I made

Dear Sir/Madam,
I am writing to request for a P45 form or P46 form if P45 had been sent already.
I have been working at xxx as a customer assistant since 31/07/07 to 05/09/2007. I was told that my P45 will be sent within two weeks after I left xxx. Unfortunately, I did not received any letter from You. If my P45 had already been sent to HM Revenue & Customs and I did not get my part of the form I will need a P46 instead.
Now I am trying to get my tax back. To do it I urgently need my P45 or P46. I would be grateful if you could send me one of those documents to as soon as possible.

to Feebs11 : and what if they had sent P45 and I didn't get it? they can't give me a copy (that's what somebody told me - is that true?) maybe you know what to do in that case?
