Could you please check my writing for taking leave

Dear sir,

My sister, Mu Mu who would like to take 6 days leave from 12/01/2009 to 17/01/209 inclusively as we are going to trip to xxx city where her father works.

Actually, i was not with them on the trip. My sister and my mom went there and my mom asked me to write a leave letter on behalf of my sister.
