Could you help me with a grammar check for one sentence?

hi, guys!

English is my second language, so I wanted to ask whether this sentence sounds okay (or do I need to make any changes Grammar-wise):

We kindly request to provide the Court until December 1, 2014 with information about job relationships and paid taxes, as well as about benefits received from December 23 up to the day when such information is requested in the United Kingdom of the defendant NAME, SURNAME

I did not know where to put that "until December 1, 2014". Does this looks okay?
