Could someone please find grammatical errors in the following letter that i am writing in response to a letter related to missing documents.?

Dear Sir/Madam,

I am writing this letter in regards to the letter that I received on March 10th, 2016, from Canadian High Commission, Immigration and Medical Services Division, London, UK. In the letter it was stated to send medical results for my spouse Ahmed Ali, who was sent medical instructions on the 16th November 2015 to undergo a medical examination. However, my husband had already undergone medical examination on the December 6th 2015 at the IOM Islamabad Migration Health Unit, which is a designated and approved center by Canadian Immigration as mentioned on CIC (Citizenship and Immigration Canada) website under panel physicians for my country and city.

Upon contacting IOM Islamabad Migration Health Unit, they informed us that my spouse’s medical examination result had already been updated in the software and sent to you, and asked us to provide you the client receipt for my husband’s medical examination so that you can re-check it.
