Hereby I inform that on August 27, 2010 seeking for consultation and recoomendations Centre for Applied Psychologies was visited by Jane Smith, personal identity number XXXXXX, declared place of residence XXXXXXX, and their grandchildren Jimmy Smith, personal identity number XXXXXX, and John Smith, personal identity number XXXXXX, in order to evaluate the psychological and emotional condition of the children, their relationships with her and with their mother, as well as in order to get recommendations.
