I think "I do not understand what you are saying at all" is this correct, because it is in present tense.

Yes, I am not understanding' is considered substandard by some people, on the basis that you either understand or you don't, ie that understanding is not a process that takes time.

I'm not so sure about this myself, and the sentence as originally written is not unusual.
