Correct use of the prefix ex

Could someone advise the correct use of the prefix ex when used to modify a noun. For example, what are the correct forms:

ex miner or ex-miner
ex policeman or ex-policeman
ex teacher or ex-teacher

If both are correct, which are preferred?
