I would say: I will be going to my home town today. I came in at 6:30 this morning, so, if it is possible, could I leave by 3:30 this afternoon? Thank you.

As I am leaving for my hometown at 3.30 PM today,I reported to work at 6.30 AM. Please give me permission to leave at 3.30 PM.

( The arrangement of ideas sounds a little odd to me. It seems that you knew that you if you reported to work at 6.30 you could leave office at 3.30. Then, you are asking your manager to permit to you to leave at 3.30(?))
